Wedding Reception Forms
1) Please print and bring the following to your first consult with Global Spinners: "Wedding Music Agenda" (Attempt to fill out as much as you can with PENCIL first, not pen. Use pen or type on the document only when you have made your final decisions.
2) When the final draft is complete, you will fill a printed copy out in pen and sign it at the bottom and submit it to us. This can be done in person, scanned into an email, or mailed. At this time we do not have a fax machine but when we do we will be posting that information in our "Contact Us" section of this website. It is extremely important that we have the same copy as you by the time of your event. This way we can prepare properly and store the song information into our computer database. If we do not have your final draft prior to your event, or if you make changes to your final draft without notifying us, we will not be able to confirm that we have the exact song choices you will be needing. Please contact us if you have any trouble downloading these forms.
Recommended: For PC users, download and SAVE the file to your computer then re-open the file in Microsoft Word or Works. These files were created in Microsoft Word, thus, will work best in this program when editing the form from your computer. Another option would be to print the blank form, fill it out in pencil, and either scan it and send it back to us, or submit it to us in person when we meet with you.
Why use pencil? Because you will most likely change your mind several times before your event, and re-printing form after form will waste ink, paper, and will be time consuming for you.
